Throughout the course you will be building a health and wellness plan for a fictitious person. In this activity you will be choosing the specifics about that person. The characteristics you choose will form a profile that you will use for your individual assignment work over the rest of the term.
You will pick from multiple categories to create your person, including giving him/her a name. Make sure you have created a profile for a person that you are comfortable creating a health and wellness plan for. You will be working with this profile for the rest of the course. No one will create a profile for a perfectly healthy person so I encourage you to work outside of your comfort zone, but do not take on too much.
Use your interests to develop the profile, as that will make the terms activities more enjoyable for you.
Every category is mandatory. Follow the directions for each question. Be sure to review your answers before you submit the form. You will not be able to make changes once the form is submitted. (PLEASE NOTE: Before filling out the form, you must first download and save the form to your computer. You can do this by clicking on the download icon located at the top right corner of the form. When you have completed the form, once again save it to your computer. You will then attach it to the dropbox for submission.) When you are ready to begin, go the Health & Wellness Case Study Creation Form.
This activity is not graded but must be submitted during Module 2. Failure to submit this assignment will result in an automatic reduction of 5 points from your first graded assignment, Case Study Part 1: Developing a Nutrition Plan, also due at the end of Module 2.
Compose your work using a word processor (such as Microsoft Word, etc.) and save it frequently to your computer. Check your work and correct any spelling or grammatical errors. When you are ready to submit your work, click Submit Assignment in the upper right corner. Click on Browse, browse your computer, and select your file. Click Open and verify the correct file name has appeared next to the Browse button. Enter your comments, if any, in the Comments area. Click on Submit Assignment.
