Occupational therapy discharge report

1. A comprehensive occupational therapy discharge report. Ensure you include the following information in your discharge report:
i. Assessments and/or outcome measures conducted, results and implications of findings
ii. OT intervention strategies and outcomes
iii. Occupational therapy recommendations/plan
References should be included in your report and a separate page of references attached to the OT Discharge Report. References should include a variety of sources, such as high quality evidence from systematic reviews and peer reviewed journal articles as well as book chapters and website references.
2. Write a one A4 page referral letter to the community OT who will be required to follow up with your patient once he/she is discharged home. Please note: you will attach the OT Discharge Report to the referral letter.
3. Prepare a package of information for your patient and their family/carers in relation to your equipment recommendations for safe discharge home. This should include at least one piece of equipment for:
i. Eating;
ii. Showering, bathing or toileting;
iii. Seating or positioning, including bedding (e.g. hospital bed and mattress); and
iv. Wheelchair for community mobility
